If you prefer to use the downloadable forms, here are the steps: 1) locate and save the appropriate form(s) to your desktop; 2) open the file and fill in the information using the fields provided; 3) print and verify application is complete before mailing.

NOTE: In order to access the pdf files you will need Adobe Acrobat Reader (version 5.0 or higher) installed on your computer.

Fingerprints: Any applicant who does not hold a valid Washington certificate at the time of application must be fingerprinted for a state and national background check. We urge applicants to submit fingerprints as soon as possible. To submit fingerprints electronically in person (no inked card is required, and the process is often much faster) contact an ESD to arrange for scanning of fingerprints. 

If you are unable to come in person, you can request fingerprint cards by sending an email with your name and mailing address to cert@k12.wa.us.

Fee Payment: Fees are not refundable. Please ensure you select and submit the correct application as the OSPI $33 fee will not apply toward a future or corrected application and will need to be paid with each application submitted. Refund Policy

Career and Technical Educator Certificates

  1. Request for Replacement CTE Certificate or
    Request for Name Change on CTE Certificate
    (for individuals who hold or have held a Washington CTE teaching certificate and have lost it, or wish to change the name on their certificate)
  2. Initial CTE Teacher Certificate Application  
    (for individuals applying for their first Initial or Probationary Washington CTE teaching certificate)
  3. Renewal of Initial CTE Certificate or
    Request for Continuing CTE Teacher Certificate

    (for individuals who hold or have held an Initial Washington CTE teaching certificate)
  4. Renewal of Continuing CTE Teacher Certificate
    (for individuals who hold or have held a Continuing Washington CTE teaching certificate)
  5. Conditional CTE Teacher Certificate
    (for individuals who do not qualify for Initial or Continuing Washington CTE teaching certificate)
  6. CTE Director Certificate
    (for individuals who qualify for the CTE director, assistant CTE director, or CTE supervisor role)
  7. CTE Counselor
    (for individuals who qualify for the CTE counselor)
  8. Occupational Information Specialist Certificate
    (for individuals who qualify for the Occupational information specialist role)
  9. Mathematics/Science Applied
    (for CTE teachers qualified to teach basic education classes)
  10. CTE Endorsement - Specialty Area
    (for those who hold a Valid Washington CTE teaching certificate and want to add a specialty area based on completion of verification of occupational experience)
